There, you're able to indicate your pet's health information, so we can fully prepare for your appointment ahead of time. There is a section to fill out if any financial assistance is needed, or if you have any additional questions.

This is a curbside drop-off appointment. Please allow for a drop off time of approximately 1 hour.

Check-in process:

When you enter the parking lot, go to the left and check in with the white booth
The person at the booth will let you know where to go under the tent
Someone will give you a call to take payment and go over any questions you may have
Payment is due at drop-off
- We take cash, debit, or credit cards
- Please be aware that credit cards may have a processing fee
- We have to have permission from the card owner to take payment
We will be coming out to your vehicle to get your pet

Pet Requirements

Please be sure to bring your pet fasted (no food after 10pm the night before). Water is okay up until the time of your appointment.
Please pay deposit and submit the pre-surgery form prior to appointment.
